Terms of Use and License Agreement

BY CONTINUING ON AND VISITING THIS WEBSITE, YOU ARE AGREEING TO THESE TERMS OF USE. DO NOT USE THIS WEBSITE IF YOU DO NOT ACCEPT THESE TERMS. BEFORE CONTINUING YOU MUST CONSENT BY INDICATING YOUR AGREEMENT WITH THESE TERMS.

ProvidersWeb is concerned that most people may not read these agreements when commencing their relationship with a website, however, it is important that you know what your commitments are, what your protections are, and that you are entering into an agreement. Please read this.

Terms of Use.
The Terms of Use may vary from time to time and ProvidersWeb retains the right to modify the Terms of Use. You may be able to find current versions of the Terms of Use on the website at any time.

Restrictions on Your use of the Site.
Although this website is designed for the Commercial Use of the subscribers for their own businesses, the contents are proprietary to ProvidersWeb and are the copyrighted product of and intellectual property rights of ProvidersWeb. By entering into this agreement, ProvidersWeb grants you a limited license to use the materials found on this site. You may use these materials for your own purposes. You may not resell these materials or provide them to others for their use. You may not modify the forms without the express written permission of ProvidersWeb. Membership does not entitle user to any ownership in the company.

Inaccuracies or Errors.
ProvidersWeb is providing you a service that gives you much information, forms, and procedures for the successful operation of an assisted living facility. ProvidersWeb is attempting to provide you with as much detailed and correct information concerning the laws, rules, and regulations that you must follow. ProvidersWeb cannot and does not guarantee the accuracy of its interpretation or presentation of the laws, rules, and regulations, which may vary from state to state and change frequently. ProvidersWeb attempts to keep you as up to date as possible. ProvidersWeb does not and cannot provide you with legal advice and render legal opinions. For this you should seek the advice of a legal professional. ProvidersWeb is presenting the content as a guide for you to follow.

State Special Forms.
ProvidersWeb has provided you with a multitude of forms, which, from experience and analysis, have proven to be beneficial, cost saving, and time saving in the assisted living world. Certain states may have developed their own forms for specific situations which must be used in that state. You should be aware of your own state requirements.

Protectability of Development
To protect yourself from lawsuits, ProvidersWeb has developed many forms for internal use. Self examination, through the use of forms, is a good way to prevent events from occurring that might lead to lawsuits. The effectiveness of these forms might be diminished if disclosure in a lawsuit were required by the Court. The law varies in this regard from state-to-state and from situation-to-situation. You should be aware that there may be issues of disclosure in a lawsuit for your attorney to resolve. ProvidersWeb cannot guarantee what documents would be protected from disclosure, but does attempt to give you insight into that issue and, by that, even prevent you from mistakes that lead to litigation.

Refunds.

No refunds are given if five (5) or more documents have been downloaded by the subscriber. Ebooks are not considered single documents, since they contain many documents. If you download an ebook, you will no longer be entitled to a refund.

Privacy and Personally Identifiable Information.
This is governed by the Privacy Policy available on the Website under privacy policy and incorporated in this agreement as modified from time to time. Personally Identifiable Information will be protected, shared and disclosed as set out in the privacy policy.

ProvidersWeb will not disclose Personally Identifiable Information to any others unless they have agreed to be bound by ProvidersWeb’s privacy policy.
